Chronic Disease Management Plan Q&A
Q: Who does the chronic disease management plan affect?
A: Employees who are EBSO (hospital) insurance holders and their spouses/dependents.
Q: Why was the chronic disease management plan implemented?
A: For your health and wellbeing. Having a chronic disease management plan helps you maximize your EBSO plan benefits and get the full advantage of recommended testing and treatment.
Q: What happens if I’m diagnosed with a chronic disease?
A: You and your physician will receive a letter and a management plan for your condition from EBSO.
Q: What is my physician’s role in my disease management plan?
A: Physicians have been asked to encourage identified patients to use the disease management plan and follow the recommendations.
Q: What health benefits are available to EBSO holders?
A: You’ll have access to a nurse navigator that will help you enhance your self-care, identify warning signs, access resources, and encourage you to adhere to your chronic disease management plan. Full Engagement Training (F.E.T.) and Diabetes Education Manager, Sandra Grunwaldt, will be available as a resource for more information on the minimum annual care recommendations. You can also use Healthier You, a health assessment tool provided by EBSO.
Q: What health resources are available to employees who don’t have EBSO insurance?
A: You can use the general chronic condition management list that was mailed to you as a guide. You also have access to Full Engagement Training (F.E.T.) Classes and Rechargers, 24 hour employee gym and tutorials, healthy food options in Café Que Pasa, free annual blood screenings, annual employee wellness month activities, annual flu shots and other vaccines, Lifestyle Balance/National Diabetes Prevention Program, diabetes education, and more.
Q: What if I don’t have a chronic condition?
A: You have access to all the plan benefits aside from receiving a specific disease management plan.
Q: What if my spouse or other dependent is on my insurance and is diagnosed with a chronic disease?
A: They will also receive a letter and a disease management plan from EBSO, and have access to the same resources you do.
